FREQUENCY VA LIABILITY + MEDIA WAIVER
By submitting this form and attending the Frequency VA Event, I agree to the following:
Liability Release:
I acknowledge that participation in retreat activities (including but not limited to music production, live performance, indoor gatherings, and traveling) involves some level of risk. I voluntarily assume full responsibility for any injury, accident, or loss I may sustain. Frequency VA and its organizers will not be held liable.
Media Release:
I grant Frequency VA permission to use photos, videos, or audio recordings of me taken during the event for promotional, marketing, and archival purposes. This includes but is not limited to social media, website use, and future promotional materials.
Code of Conduct:
I agree to respect all participants, property, and guidelines set by the organizers. Any behavior that puts others at risk or negatively impacts the experience may result in my removal from the venue without refund.
Creative Collaboration:
I understand that any music, visuals, or projects during the event may be used by Frequency VA in future releases or promotional campaigns, and I agree to participate in good faith, contributing positively to the process.
Non-Refundable Policy:
All bookings are final. In the event of cancellation or inability to attend, I understand that payments are non-refundable but may be transferred to another approved participant at the discretion of the organizers.
